How to create a web Form in Google Drive
Google Forms is a useful tool to help you plan events, send a survey, give students a quiz, or collect other information in an easy, streamlined way. In this lesson we will show you how to create a simple questionnaire and send it through e-mail.

Click CREATE and select Forms.
|
|
|

Enter the title and select any theme you want. I chose default theme.
|

Enter the first question in the question Title field and select Required question. Confirm with Done.
Required question means that it will be necessarily to answer the question.

At question type you can choose in what form the answers will be given. I chose Text.
|

To add another question click Add Item.
|

Enter the second question and click Done.
|
|
|

Now we will add question with two possible answers.
Add new question, click on the box next to Question Type and select Choose from a list.

Enter the question and write possible answers below.
Click Done.

To save some time we can duplicate the question.
Click Duplicate icon.
|
|

Question is duplicated. Now just enter the new question with new answers. This time I added three possible answers.
Click Done.

The questionnaire is done.
|

Now we will send questionnaire through e-mail.
Click Send form.

Above you have a link to share the questionnaire and below you have option to send questionnaire through e-mail. We will send questionnaire through e-mail.
Enter addresses to the address field and click Send.

After sending the questionnaire you can also choose response destination, where your answers will be collecting. I chose New spreadsheet.

You can see the statistics under View responses.
